When Horace Samuel Ensign Jr. was born on 10 November 1871, in Salt Lake City, Salt Lake, Utah, United States, his father, Horace Sobeski Ensign, was 23 and his mother, Martha Triplett, was 19. He married Mary Linda Whitney on 21 June 1894, in Salt Lake City, Salt Lake, Utah, United States. He lived in Helena, Lewis and Clark, Montana, United States in 1920 and Wichita, Sedgwick, Kansas, United States in 1930. He registered for military service in 1895. He died on 29 August 1944, in Salt Lake City, Salt Lake, Utah, United States, at the age of 72, and was buried in Salt Lake City Cemetery, Salt Lake City, Salt Lake, Utah, United States.
